This Survey questionnaire is prepared for collection of information for the multivitamin suppment- Supradyn Daily. By submitting this survey, you are authorizing Bayer to store, process and use your personal data as well as data about your Survey into a healthcare professionals’ data base for internal processing, including the transfer of such data to the Bayer Group Companies and any service providers of Bayer for maintaining the data base for review and processing purposes, and for conducting analysis along with a fully independent agency. Your Survey inputs will be further used to improvise and enhance our products and may also be utilized in building and strengthening health literacy and education. The information collected through this Survey is for internal research, reference and promotional purposes only, and shall not be utilized for any purposes other than outlined above. This Survey by no means obligates or influences you to prescribe, recommend or purchase any products from Bayer or any of its affiliates.
Both parties are independent contractors, and nothing in this Agreement creates any partnership, joint venture, agency, franchise, sales representative or employment relationship between us. You may not make any statement, whether on a site maintained by you or otherwise, that would contradict anything in this section. You are solely responsible for the reporting and payment of any taxes for money earned for completing the survey.
We reserve the right to refuse credit for a survey for a number of reasons, including, but not limited to, fraud, system abuse and early termination. You are doing the survey on a voluntary basis. Payment from surveys will only be made after verification that surveys completed have been validated.
We agree not to use any of your confidential information for any purpose except in accordance with this agreement. We agree not to disclose any of your confidential information to any third party other than to our employees and consultants who are bound by confidentiality obligations and are required to have access to the confidential information in order to operate the site and services.
You will be reimbursed for successfully completing the survey after receiving the survey responses. Reimbursement will be made based on the completion of the survey; incomplete surveys will not reimbursed. You need to give us your bank account information (cancelled cheque & pan card copy). All of the data we receive from you will be treated with utmost confidentiality and will be utilized only for the purposes of making payment on successful completion of the survey. Most payments are verified and fulfilled within 30 days, pending account validation. In some cases, payments and orders may require additional time to be processed. Before payment, accounts are audited for validation, to ensure that no fraud has taken place and that the information provided by an individual is current and correct. We cannot make payment in cash or through orders to any member with incomplete or inaccurate account information.
The use of the website and these terms is governed by the laws of India. Any disputes which arise relating to the survey link or in connection with the survey link will be subject to the exclusive jurisdiction of the courts in Mumbai, India.
